After arriving at our office we will give you the FAA Form 8500-8. It will take 15 to 30 minutes to complete, so please allow for that prior to your scheduled appointment time.

For your convenience, and with advance notice, we offer a pickup / shuttle service from the airport, Mettetal "1D2". Our office is less than 1 mile from the airport.

1. Picture I.D. -Drivers license or Passport
2. Eyeglasses, if used.
3. Special Issuance Letter, if applicable, with pertinent medical information
4. Medical records from personal physician regarding any chronic conditions such as asthma, high blood pressure, etc.
5. List of all medications you are using. Please note the name, dosage, length of time used and the condition requiring the medication.
6. A SODA (Statement Of Demonstrated Ability) if applicable
