1st, 2nd, and 3rd Class Flight Physicals are $100.00. EKG (if applicable) $65.00. Special Issuance consultations, D.O.T., Various Auto and Boat racing, Merchant Marine, ETC. exams are $95.00. Assistance in obtaining Special Issuance, additional exam or treatment for a medical problem at the same time as a physical is $50.00 for each additional service. Insurance Form (FMLA, disability, ETC.) or other form completion is $25.00. Payment is due at the time of service we accept cash, personal check, Debit Card, Visa, MasterCard or American Express. We do not accept insurance assignments for your exam, but we supply a coded receipt which you can submit to your insurance for possible reimbursement.

After arriving at our office we will give you the FAA Form 8500-8. It will take 15 to 30 minutes to complete, so please allow time. You can also go to “Medxpress.faa.gov” and complete this form online before your appointment. Please print off a copy of the exam and the confirmation number and bring it to the visit. We will need the number to import your exam and see it in the system.

For your convenience, and with advance notice, we have an arrangement with the Clarion Hotel on Merriman Rd. for shuttle service to and from the office. Please refer to the information and link on the home page.

Please help us help you by using this checklist to insure that you are prepared for the exam.

1. Picture I.D. -Drivers license or Passport
2. Eyeglasses, if used.
3. Special Issuance Letter, if applicable, with pertinent medical information
4. Medical records from personal physician regarding any chronic conditions such as asthma, high blood pressure, etc.
5. List of all medications you are using. Please note the name, dosage, length of time used and the condition requiring the medication.
6. A SODA (Statement Of Demonstrated Ability) if applicable
